TIPS AND TRICKS IN ELECTROCARDIOGRAPHY:CLINICAL CASES
Mariana Floria Assistant Professor, MD, PhD
University of Medicine and Pharmacy „Gr. T. Popa” Iasi
How an electrocardiogram (ECG) is performed and how it is interpreted can lead sometimes to problems. Frequently ECGs could be interpreting in a non-threatening and simple way if step by step analyzing in clinical context is done. Sometimes surface ECG could be challenging without other data about clinical context and sometimes without biological or transthoracic echocardiography data. Therefore an integrative approach in these cases is essential. Sometimes timing and sequential interpretation is the key. The emergency physician should be aware of such issues so he does not embark on an unnecessary diagnostic or treatment pathway.
This workshop will be about some tricks and tips on surface ECGs presented during some clinical cases. They might be saving solution in life-threatening situations. In addition these cases will underlie the importance of data integration in clinical practice. This workshop will be about tachycardia with narrow and large QRS from differential diagnosis to treatment and from antiarrhythmic to proarrhythmic effect.

Radiation therapy is a part of the multidisciplinary treatments for cancer. It uses high-energy particles or waves, such as x-rays, gamma rays, electron beams, or protons, to destroy or damage cancer cells. Radiation can be given alone or used with other treatments, such as surgery or chemotherapy. Oncological drugs can make cells more sensitive to radiation. Using advanced imaging technics like CT, MRI and PET-CT we can accurately locate the tumor volume and the radiosensitive organs. Modern radiotherapy technics allows us to irradiate the tumor volume with higher doses in order to kill malignant cells without major damage of healthy tissues. The aim of the workshop is to present the rolls of multidisciplinary team in cancer patinent management and the patient pathway from the initial consultation to the end of the treatment in the radiotherapy department. The students shall have the opportunity to identify and delineate the radiosensitive anatomical structures and the tumor target volumes using CT simulation.

THE ART OF MOLDING A DISABLE FOOT TO CREATE A NORMAL LIFE – PONSETI METHOD FOR CONSERVATIVE MANAGEMENT OF CONGENITAL CLUBFOOT
Assistant Professor Carmen Iulia Ciongradi MD, PhD
Teaching Assistant Ioan Sârbu MD, PhD
Pediatric Surgery department, University of Medicine and Pharmacy „Grigore T. Popa” Iași
Congenital clubfoot (talipes equinovarus foot) is a well known and one of the most commonly referred problem in pediatric orthopedics, characterized by a complex three dimension malformation of the foot, in which one or both feet are excessively plantar flexed, with the forefoot swung medially and the sole facing inward. The congenital foot deformity affects the bones, muscles, tendons, and blood vessels of one or both feet. The prevalence of clubfoot is relatively high, 1 to 3 per 1000 live births in Caucasians. The treatment of clubfoot is still one of the big challenges in pediatric orthopedics. Clubfoot it’s a malformation known from the ancient time as it was identified in the wall paintings found in the ancient Egyptian tombs. Its treatment was described in India as early as 1000 B.C, but it was Hippocrates who gave us the first written description of the deformity, as well as the descriptions of the manipulative correction remarkably similar to current non-operative methods. Unfortunately, Hippocrates ideas and principles of conservative treatment in congenital club foot were apparently forgotten by next generations, and for centuries children bearing this malformation underwent different types of surgical procedures on the skeleton or muscles of the ankle and foot. Surgery in club foot is invariably followed by scarring, stiffness and muscle weakness. The modern era in the treatment of congenital clubfoot started with Kite’s work, completed by Ignacio Ponseti who developed a method of conservative treatment based on the understanding of the functional anatomy of the foot and of the biological response of bones, muscles and ligaments to gradually position changes by manipulations and casting. The aim of this workshop is to present the principles behind the conservative, modern treatment of congenital club foot. During our hands-on session students will have the opportunity to apply the Ponseti principles on training models (manipulations, casts, tenotomy bracing, etc). So let’s embrace together the delicate art of sculpture a normal foot from an abnormal one!

ANATOMO-MACROSCOPIC AND IMAGISTIC STUDY OF ANTERIOR CEREBRAL ARTERY
Teaching Assistant Alin Horatiu Nedelcu MD, PhD
University of Medicine and Pharmacy „Grigore T. Popa” Iasi
Anterior cerebral artery is the medial terminal branch of internal carotid artery, with a large distribution on the medial aspect of cerebral hemisphere. The two anterior cerebral arteries are connected by anterior communicating artery.Together form the rostral part of Circus ArteriosusCerebri, a circulatory anastomosis between carotid and vertebral arterial circulations.
The function of the anterior cerebral artery is to supply important deep structures of the brain such as: optic chiasm, hypothalamus and the head of the caudate nucleus, internal capsule and putamen. The superficial branches distribute to medial aspect of fronto-orbital, frontal and parietal lobes. They also spread parasagittal on the lateral aspect of the hemisphere for about 1-2 cm.
The aim of this workshop is to raise awareness on anterior cerebral artery as one of the main arterial source of the encephalon, to dissectthe formalized human brains and to identify its central and superficial branches.
The workshop consists in two parts:
- Theoretical part: it includes a short overview on the anterior cerebral artery, the description of this artery, the limits and segmentation, the collateral branches and the morphologic variants communicated in medical literature.
- Training part: in order to identify the anterior cerebral artery, we will dissect formalized human brains with the aid of an operator microscope (OPTRON – Zeiss OPM 6). During the dissection, we will highlight the origin, the segments and the superficial and central branches. We will give special attention to recognition of morphological variants.

CARDIAC SOUNDS AND MURMURS – PRACTICAL APPROACH
Teaching Assistant Bogdan A. Stana MD, PhD
2nd Pediatrics Clinic, „Gr. T. Popa” University of Medecine and Pharmacy, Iasi, ROMANIA
Auscultation is one of the golden-four elements of the clinical exam, along with inspection, palpation and percussion. Depending on anatomic location, auscultation can provide essential tips for diagnosis (positive and differential).
Cardiac auscultation offers lots of information about the status of the heart muscle, valves of the heart and the great vessels, turbulences made by the flow of blood in physiological and pathological conditions. It all begins with understanding the production of the cardiac sounds in the normal heart. Then, using the stethoscope and different techniques, pathological conditions can be clinically identified and discussed. From the practical point of view, a „trained ear” is prone to a faster identification and diagnosis of pathological conditions.

MAKING SENSE OF SPIROMETRY
Dr. Irina Alexandru MD
Allergy and Clinical Immunology
Spirometry is a diagnostic tool; the most common of the lung function tests. It is used to detect airflow obstruction, which is a characteristic of certain lung diseases, such asthma and chronic obstructive pulmonary disease. In these diseases, evaluating lung function with spirometry is important for diagnosis and monitoring, as recommended by most guidelines.
It can detect very subtle changes which may not be obvious on physical examination or even to the patient. Through routine spirometry, many abnormalities can be detected in the early stages, when treatment is usually most effective. Spirometry can also be used when testing the reversibility of the airways to a bronchodilator drug.
The workshop outcomes are for the students to learn why respiratory function tests may be required. The students will have the opportunity to receive training. During the practical session, we also hope to use the opportunity to ask the students to develop their communication skills by working in pairs or triads, using role play to ‘act out’ the patient/health care professional relationship.
